If you are an approved captive manager that submitted the captive manager application online, your login information is the same as the login information you established when you created the online captive manager application.

If your captive manager application was completed on paper and you have not yet obtained your login information, please contact Kathy Hart at (919) 807-6180 or captives@ncdoi.gov to obtain your credentials.
